Client Roster Reports
Client Roster Reports are a requirement in some states for licensure surveys. These documents compile data from a number of sources within Residex, allowing the surveyor to view data about resident needs and acuity - and then make decisions on which resident records they may choose for closer review.
This document describes settings and requirements that are necessary for the Client Roster report to populate data accurately. The example below shows the Minnesota Client Roster. Other state roster reports are similar.
Note the following:
- The resident names are located on the Y axis, and are hyperlinks - allowing you to 'click through' to see a report with each resident's detailed information.
- The parameters required by the state are listed above the table on the X axis; each contains a number in parentheses. Use the key at the bottom of the report to interpret where the data pulls from (e.g. from incidents, services, assessments, etc...).
- Note that there may be data that must be manually entered. This is denoted with a (9) on this report
- Start of care is calculated by the date of the first care service. There is a similar report, 'Client Roster - MN - SOC' that calculates based upon the intake date of the resident.
To walk through the requirements for this report:
- Housing only - reflects a resident with NO services at all in their service plan.
- Tube feeding - from assessment item ADL > Eating Needs/Feeding > Requires use of a tube or parenteral or intravenous instruments to be fed.
- Housekeeping/Laundry/Meals - from resident services. If the resident ONLY has these services, this box would be checked for the resident.
- Memory Care - if the resident resides in an community that is flagged as a memory care setting.
- Hospitalizations - if the resident was placed "on hold" and the reason "hospitalization" was selected in the prior 3 months.
- Falls - if the resident has had fall Incidents entered in the previous 3 months.
- Bed Rails/Restraints - from assessment item Safety > Bed Safety > Bed rails in use, include description and condition of bed rail.
- Skilled Nursing - must be manually entered
- Hospice - if the resident has a hospice provider entered as a Resource - and that hospice provider is 'active'. If the resident no longer is in hospice, the resource should be made inactive.
- Nebulizer - will be checked if the resident has a medication administered by the route 'nebulizer'.
- Psychotropics - if the resident has a med in their current med list that is flagged as a psychotropic. 411 users are able to edit medication details in the Global Medication list (marking or un-marking psychotropics).
- Med Admin - if the resident has a Med Admin service that is designated in the Global Service List as being in the category of Med Assist. NOTE: Contact Residex support team members before making any changes to this.
- Med Setup - if the resident has a Med Setup service that is designated in the Global Service List as being in the category of Med Setup.
- Insulin - if the resident has a medication in their current med list that contains the term 'insulin' (either in the trade name or a generic name). Users with the 411 Role are able to edit medication entries to ensure insulins are flagged here.
- Dialysis - from assessment item Independent Living > Other Services Needed > Dialysis
- Oxygen - if the resident has a service with the term 'Oxygen' in the title (e.g., oxygen delivery, oxygen administration, or oxygen maintenance) and in the Global Service List, that 'Oxygen' service Category is set to Oxygen.
- Ventilator - is manual entry
- CPAP or BiPap - from assessment item ADL Needs > Respiratory Equip. > CPAP / BiPap
- PT/OT/ST - from assessment item Independent Living > Other Services Needed > PT / OT / Speech Therapy
- Blood Sugar Tests - from current services that are in listed in the Global Service list as included in the category 'Blood Glucose'. Users with the Administrator (Role 411) can edit this detail.
- Catheter - pulls from resident services with the term 'catheter' in the title.
- Wound Care- pulls from resident services that are identified in the Global Service List as being in the 'skin' category.
- Treatments - if the resident has a treatment service AND its corresponding Supervision service included in their service plan.
